Sutton’s integrated health and social care pilot shows benefits for patients

In a nutshell:  A six-month pilot targeted people who had presented at A&E or passed through A&E with heart failure, diabetes or COPD. Sutton Council installed monitoring devices in each patient’s home so GPs could check their client’s blood pressure, blood oxygenation and other indicators, then take early action.  Each client’s needs were also discussed at weekly meetings by a social worker, psychologist, GP, community matron and district nurse.

An integrated care facilitation model improves quality of life and reduces use of hospital resources

In a nutshell: Describes a Melbourne consortium of community based health care providers formed to reduce the demand on hospital emergency services and improve health outcomes for patients with COPD and chronic heart failure (CHF). The model of care utilised ‘Care Facilitators’. The outcome measures used suggest that an integrated care facilitation model that is patient focussed, provides an education component to promote greater self-management compliance and delivers a continuum of care through the acute and community health sectors, may reduce the utilisation of acute health care facilities and benefit the patient.

Development of an integrated COPD service model in an inner-city region in the UK

In a nutshell: A report of initial findings and 12-month results following a health needs analysis and benchmarking exercise in Salford.  The authors conclude that the integrated COPD service model described has been successful in increasing diagnosis, reducing hospital admissions and reducing length of hospital stay.

A pilot study of identification and case management of high-risk COPD patients in a general practice

In a nutshell: The authors examine the effect of case management in primary care of patients with COPD at high risk of hospital admission, identified using a novel multidimensional index of disease severity (DOSE index). This observational pilot study in a single general practice showed case management of high-risk patients in primary care may reduce hospital admissions. This needs to be tested in a randomized controlled trial.

Pulmonary Rehabilitation: what we know and what we need to know

In a nutshell: Looks at the future direction of pulmonary rehabilitation; where it fits into the all round care of the COPD patient; and how clinicians can best apply this intervention. It concludes that although PR is effective for other chronic respiratory diseases, the focus is COPD since most of the evidence base and patient referrals are for this disease. The article provides insight into 3 broad areas appropriate of implementation: positioning PR in an integrated care model for COPD patients; improving the effectiveness of this intervention; and expanding the recognition, application, and accessibility to PR.
